INNOVATIVE WELLNESS CUISINE award application must include:

1.1   Company Register

1.2   Registration and/or Notification from the Ministry of Public Health (as a Spa or a Food Service facility)

1.3   Full name and contact details of an authorised company representative

1.4   One (1) spa cuisine menu of not less than 10 food items, which clearly shows relevant nutritional information

1.5   Three (3) standard recipe cards, including colour photo and nutritional information as listed on menu.  Must including one salad and one dessert item.

1.6   Copy of Staff Training Plan
